California Privacy Rights

Legally required disclosures under the California Consumer Privacy Act (CCPA). We have tried to explain our practices in plain English throughout, and nothing here should come as a surprise.

Overview

This California Consumer Privacy Act Notice supplements our Privacy Policy and applies specifically to California residents. It describes the categories of personal information we collect, why we collect it, and the rights you have under California law.

We believe everyone deserves clear, honest answers about how their data is used. While the legal requirements here are California-specific, the privacy tools described below are available to all users regardless of where they live.

Your California Privacy Rights

Under the CCPA, California residents have the following rights regarding their personal information:

  • Right to Know. The categories of personal information we have collected, the sources we collected it from, the business purposes for collecting or sharing it, and the categories of third parties we share it with.
  • Right to Know Specifics. The specific pieces of personal information we have collected about you.
  • Right to Delete. To request that we delete personal information we have collected from you, subject to certain exceptions permitted by law.
  • Right to Correct. To request correction of inaccurate personal information we maintain about you.
  • Right to Opt Out. To opt out of the sale or sharing of your personal information. We do not sell personal information in exchange for monetary consideration, but certain disclosures may qualify as a sale under California law.
  • Right to Limit Sensitive Data Use. To limit our use of sensitive personal information to purposes permitted under applicable law.
  • Right Against Discrimination. We will not discriminate against you for exercising any of these rights. You will not receive a different level of service, pricing, or quality as a result of making a privacy request.

California Shine the Light Law

Under the California Shine the Light Law, California residents who provide personal information in obtaining products or services for personal, family, or household use are entitled to either a list of the categories of information shared with third parties and the identities of those third parties, or a mechanism to opt out of such sharing.

We have elected to provide an opt-out mechanism. To opt out of having your information shared with third parties, please send an email to privacy@forlifenhealth.com with the phrase opt out in the subject line, or contact us by mail at the address below. You may also discontinue any marketing communications from us at any time using the unsubscribe link included in each email.

How to Submit a Privacy Request

To exercise your rights under the CCPA, including requests to know, delete, correct, or opt out, please contact us through one of the methods below. We will acknowledge your request within 10 business days and respond within 45 calendar days, or notify you if we need additional time as permitted by law.

  • Email. Send your request to privacy@forlifenhealth.com with a clear description of your request.
  • Identity Verification. We may ask you to verify your identity before processing your request. This is to protect your information from unauthorized disclosure or deletion.
  • Authorized Agents. You may designate an authorized agent to submit a request on your behalf. We may require written authorization or proof of power of attorney and will verify the agent's authority before processing the request.

Changes to This Notice

We review our privacy practices regularly to ensure they remain accurate and compliant. When we update this notice, we will post the revised version on this page with an updated effective date. We will not change this notice in a way that meaningfully reduces your rights or significantly alters how we use or share your data without notifying you in advance and giving you an opportunity to discontinue our services if you choose.
